Program Elegibility

Children under age 19 who are a U.S. citizen, U.S. National or Qualified alien; reside in PA; meet income requirements; and are uninsured and not eligible for PA Medical Assistance.


Applicant must provide: proof of income; birth date and social security number of applicants; family day care expenses (if applicable); family work commuting expense (if applicable); and private health insurance information (if have or had within last 6 months.

Application process

Apply online, download an application from the program's website, or apply over the phone by calling the toll-free number listed above. If you apply by mail, mail application to insurance company you have selected to cover you under CHIP (see website for list of CHIP insurance companies).


Checkups, sick visits, prescription drugs, emergency room care, hearing tests and hearing aids, immunizations, dental care, vision testing and eyeglasses, lab tests and x-rays, mental health and substance abuse treatment, durable medical equipment, hospital care. Based on income, care is either free or low cost.


Health insurance program for children without health insurance whose families are not eligible for Medicaid because income is too high.
